Community Link Worker

The surgery now has a Community Link Worker who specialises in Social Prescribing. Her name is Carrie Horton.

What is social prescribing?

Social prescribing is part of a commitment to personalised care

Personalised care means that our patients all have a choice and control over the way their care is planned and delivered, based on ‘what matters to me’ and individual strengths and diverse needs. This happens within a system that makes the most of the expertise, capacity and potential of people, families and communities in creating better health access, outcomes and experiences. Personalised care takes a whole-system approach, integrating services around the patient. It is an all-age model, from maternity and childhood through to end of life care, encompassing both mental and physical health support. It can contribute to advancing equality and reducing inequalities in access and outcomes for all.

Surgery Closed for Half day Training

The Surgery will be closed from 12:30pm on Wednesday 10th June 2020 for staff training. If you need urgent medical advice between 12 noon and 8am the next morning please ring NHS 111 and after 8am please ring 07946605039.

Bank Holiday Closure

The surgery will be closed on Monday the 25th of May 2020 due to the Spring Bank Holiday and will not reopen until 9am on Tuesday the 26th of May 2020.

If you need urgent medical advice between 6pm on Friday the 22nd of May 2020 and 8am on Tuesday the 26th of May 2020 please ring NHS 111 and after 8am on Tuesday the 26th of May 2020 please ring 01207 561736 and you will be directed to the on call GP.

Coronavirus COVID-19

Due to the Coronavirus outbreak, NHS England has given national and local guidance that patients should not attend the surgery unless given an appointment. This is to ensure the safety of the community, patients and staff.

If you have a cough or fever and are well, self-isolate for 7 days. If you need more advice please call 111.

When telephoning the surgery you will be asked questions by reception staff to direct you to the best place for care. This may be a telephone consultation with the GP or Advanced Nurse Practitioner.

Patients with online access can use the service to book a telephone consultation and request prescriptions. Patients can also use e-consult which is available via the practice website at www.chopwellmedicalpractice.nhs.uk

There is an external prescription box located at the main car park gates for prescription requests which will be checked two times a day.

If you are asked to attend the surgery please listen to the instructions you are given carefully.

We appreciate your understanding in these pressured times.

Car Park
