How to register

If you would like to register at the practice you will need to complete a registration form. Along with the GP registration form you will be given a new patient questionnaire, (this will include questions on your height, weight, if you smoke, current medication, alcohol intake etc.).

Acceptance onto the surgery’s list is as at the discretion of the partners of the surgery. You will also be asked for proof of identity. (Please refer to our registration policy).

Summary Care Record.

As part of your registration you will be asked if you wish to have a summary care record created. This is an electronic record which contains information about your medication, allergies and any adverse reactions to medicines you have had. This record is used to support emergency care and can be accessed, with your permission, by healthcare staff in emergency or who look after you when the practice is closed. GP2GP

Once your details are registered on our computer system your electronic records are requested automatically by the computer from your last GP.  Providing they have their computer set up to accept this request, your electronic records will be sent to our computer within a very short time.  This system is called GP2GP.  Please be aware that your paper medical records may take several weeks to arrive at the Practice.
